Academic Suspension Information
Criteria And Required Documentation For Filing For A Waiver Of Suspension Period On Second Or More Suspensions
Appeals For A Waiver Of Suspension Period Shall Not Be Considered Without The Following Documentaton:

Illness / Injury:

A student must provide a signed letter on official stationery from the attending physician, licensed professional counselor, or psychiatrist stating:
-
the beginning and ending dates of professional services
-
that the student is ready to assume academic responsibilities
-
that the effects of the illness/injury no longer pose a threat to satisfactory academic progress
-

A typed letter from the student stating:
-
readiness to assume responsibility for satisfactory academic progress
-
a request for a waiver of suspension period
-

Death Of A Close Family Member:
-
The student must provide one of the following:
-
copy of death certificate
-
obituary
-
letter from attending clergy
-
-
A typed letter from the student requesting a waiver of suspension period

Natural Disaster / Traumatic Event*
A typed letter from the student stating:-
the extenuating circumstances involved
-
an explanation of the undue hardships caused by the disaster/national defense/traumatic event
-
an explanation of why the conditions no longer pose a threat to satisfactory academic performance and a readiness to assume responsibility for satisfactory academic progress
-
a request for a waiver of suspension period
*Other Traumatic Event - means circumstances of a serious nature, over which the student had no control.

Copy of military orders

A typed letter from the student requesting a waiver of suspension period
