Outline for a well written appeal letter

YOUR LETTER MUST BE TYPED!!!

1. Request a waiver of the suspension period. Suspension period is one calendar year.
   
2. State the extenuating circumstances involved, or your reason for appealing.
   
   1. Illness/Injury
   2. Death of a close family member
   3. Natural Disaster/Traumatic Event
   4. National defense
      ---or---
   5. Other reasons for unsatisfactory progress
      
      See attached “Criteria and Required Documentation For Filing For A Waiver Of Suspension Period” on the reverse side of your appeal form.
      
3. Explain why the conditions no longer (or have a lesser chance to) pose a threat to satisfactory academic performance.
   
4. Provide a plan for academic success.
   
5. State your readiness to assume responsibility for satisfactory progress.
