Falsification of the application may be cause for my dismissal and may result in denial of all credit earned. I authorize Nicholls State University to request a transcript from the schools provided. I do hereby authorize Louisiana public post secondary education access to my academic records.
The Nicholls State University Bulletin is an official publication of the University governing student admissions and academic status. Admission to the University does not guarantee a student’s admission to a particular curriculum which may have additional entry standards (See University Bulletin). Please note: It is the policy of NSU that admission to the University is open to all persons, regardless of race, creed, color, age. disability, national origin, marital status or veteran’s status, who meet the admission requirements and qualifications of the University.
I have read and understand the information from the Centers for Disease Control and Prevention (CDC’s) website at http://www.cdc.gov/nip/publications/VIS/default.htm regarding vaccine preventable diseases and related vaccinations for MMR, TB and Meningococcal (meningitis). My signature indicates that I have read and understood the information. I certify that if I am a male I will, if required, register for Selective Service, available at www.sss.gov.
