National Membership Form. Please send
to the Louisiana State Olympiad organizers, including membership fee, as
soon as possible to receive coach's manual and official rule book. State science olympiad website is

Membership Form Please send, including membership fee, to
Nicholls State University. Form and fee must be received by the deadline
stated on the form. Please email Darcey.Wayment@nicholls.edu as soon as possible to let us know
of your intent to participate.

Code of Conduct Form. Please have all team
members (students), parents, and coaches sign and submit at check-in.

Event sign-in sheets. Color-coded
hardcopies --- Blue paper for division B, Canary (yellow) paper for
division C --- will be given out during registration. Students must submit
sign-in sheet to the event coordinator. Should you prefer to prepare them
ahead of time, please print the the pdf files provided here (Division B, Division C), preferably on paper with the right color.
