Appointment Information

  • An appointment is necessary.
    • Please call 985.493.2600 to schedule an appointment.
    • If you choose to walk in without an appointment, there may be considerable wait times and there is no guarantee that we can see you the same day.
    • You should allow approximately 30 minutes to 1 hour for your visit.
    • You CANNOT schedule an appointment on the Patient Portal.
    • There will be a $5 charge for any missed appointment.
  1. University Health Services Patient Portal
    UHS is currently transitioning from paper charts to electronic medical records.  All patients need to register for the UHS/Medicat Patient Portal.  Please click on the link and follow the directions to complete registration.

    • If you are a new patient, click on “Forms” and complete the following forms:
      1. “Health History Form”
      2. “Acknowledgement of Notice of Privacy Practices”
        (Please see Notice of Privacy Practices)
      3. “Consent to Treatment of Medical and Surgical Procedures.”
  • Please provide the following when you check in for your appointment:
    • Insurance/Medicaid card.  You can also email your card to healthservices@nicholls.edu.
      • All patients with insurance or Medicaid should come to the clinic with an approved medication list or formulary which is provided by your insurance company or by Medicaid.
      • UHS does NOT file any claims with insurance or Medicaid and does not participate in prior authorization of medications or procedures.
  • If it is necessary to return to clinic for a follow-up appointment, please schedule the appointment before you leave the clinic.
