Plan Details:
- Plans purchased in fall expire at the end of the spring semester
- Plans purchased in spring expire at the end of the summer session
- Non-refundable
Payroll Deduction Enrollment: To enroll in payroll deduction, complete the form available at the Galliano Dining Hall.
- 50% of the total cost is due at the time of sign-up
- The 10% payroll discount does not apply to this plan
- Deductions can only be canceled in the event of medical leave or employment separation
To cancel deductions, notify both Campus Dining Management (Galliano Cafeteria) and Human Resources Benefits Office (Elkins)
